BACKGROUND: Lactation consultants frequently advise adjustments to fit and hold (or positioning and attachment) with the aim of optimising intra-oral nipple placement. However, approaches to fit and hold vary widely, with limited evidence of benefits, and effects of fit and hold on infant tongue movement have not been examined. The aim of this preliminary study was to investigate whether a gestalt breastfeeding intervention alters tongue movement, using measurements from ultrasound imaging to determine nipple placement and intra-oral nipple and breast tissue dimensions. METHODS: Ultrasound measurements were conducted in five breastfeeding dyads, infants aged 4-20 weeks, while feeding in their usual or 'standard' position and again after brief application of gestalt principles of fit and hold. Four of the mother-baby pairs, who had received comprehensive lactation support, reported persisting nipple pain. Three of these infants had difficulty latching and fussed at the breast; three had been diagnosed with oral ties. A fifth pair was breastfeeding successfully. RESULTS: Ultrasound demonstrated that the distance from nipple tip to junction of the hard and soft palate decreased, intra-oral nipple and breast tissue dimensions increased, and nipple slide decreased after a brief gestalt intervention. CONCLUSION: These preliminary findings suggest that changes in fit and hold impact on infant tongue movement and contour. Further research investigating short- and long-term outcomes of a gestalt breastfeeding intervention in larger cohorts is required.

Família, conceito marcante para sociedade e cultura, importante alicerce das relações interpessoais, mostra mudanças e novos modelos. A decisão em ter filhos passa a ter base na experiência e realização individual, não mais por uma necessidade de modelo de família nuclear tradicional. O estudo teve como objetivo compreender a vivência de algumas modalidades de família na decisão de ter e criar os filhos. Para tal, escolheu-se a Abordagem Gestáltica como fundamentação. A Gestalt-terapia observa o processo, uma integração que nunca se completa. A presente pesquisa investigou três modelos de famílias; monoparental, casal heterossexual e homossexual. A metodologia utilizada foi pesquisa qualitativa fenomenológica. Realizou-se entrevistas semiestruturadas, com dados analisados pelo método de Giorgi. Com as entrevistas, percebeu-se que a chegada do filho traz incertezas, interrogações, novas divisões de tarefas e renúncias por parte das famílias. Conclui-se que as famílias vivem experiências semelhantes independente do seu modelo.


Family, a striking concept for society and culture, an important foundation of interpersonal relationships, shows changes and new models. The decision to have children is based on individual experience and achievement, no longer because of a need for a traditional nuclear family model. The study aimed to understand the experience of some family modalities in the decision to have and raise children. For this, the Gestalt Approach was chosen as the basis. The Gestalt-therapy look at the process, an integration that is never completed. The present research investigated three families models; single-parent, heterosexual and homosexual couple. The methodology used was phenomenological qualitative research. Semi-structured interviews were conducted with data analyzed by the Giorgi method. With the interviews, it was noticed that the arrival of the son brings uncertainties, questions, new divisions of tasks and resignations on the part of the families. We conclude that families experience similar experiences regardless of their model.


La familia, un concepto sorprendente para lasociedad y la cultura, una base importante de las relaciones interpersonales, muestracambios y nuevos modelos. La decisión de tenerhijos se basaenla experiencia y los logros individuales, ya no debido a lanecesidad de un modelo tradicional de familia nuclear. El objetivo delestudiofuecomprenderla experiencia de algunas modalidades familiares enladecisión de tener y criar hijos. Con este fin, el enfoque Gestalt fue elegido como larazón de ser. La terapia Gestalt observa elproceso, una integración que nunca se completa. La presente investigacióninvestigótres modelos familiares; Padres solteros, parejasheterosexuales y homosexuales. La metodología utilizada fuelainvestigación fenomenológica cualitativa. Se realizaron entrevistas semiestructuradascondatosanalizados por el método de Giorgi. Conlas entrevistas, se notó que lallegadadelniñotraeincertidumbres, preguntas, una nuevadivisión de tareas y larenuncia de lasfamilias. Se concluye que lasfamiliasviven experiencias similares independientemente de su modelo.

A gestalt-terapia constitui-se como uma psicoterapia dialógica, como uma clínica do encontro. Investigações empíricas sobre a interface existencialismo dialógico-psicoterapia gestáltica são escassas. Considerando a relação dialógica como eixo desta abordagem, objetivou-se compreender as experiências de profissionais de psicologia no tocante à relação dialógica em gestalt-terapia. Como recurso metodológico, utilizou-se a pesquisa qualitativa de método fenomenológico, de orientação semiótica. Foram realizadas entrevistas fenomenológicas abertas com cinco colaboradoras. Os dados foram gravados e transcritos na íntegra. A análise pautou-se na reflexibilidade fenomenológica, percorrendo pela descrição, redução e interpretação. Os resultados indicam a abertura, presença e o uso da epoché pelo terapeuta como elementos fundamentais para a relação dialógica. Experiências positivas de atitude Eu-Tu foram encontradas, sugerindo episódios culminantes deste processo. Houve confusão entre o conceito de atitude e momentos Eu-Tu, com valorização deste princípio em detrimento à atitude Eu-Isso, o que evidencia a necessidade de repensar a formação do gestalt-terapeuta. Assim, a relação dialógica em gestalt-terapia apresenta-se como um campo de pluralidades fenomênicas. Cabe ao profissional um andar cuidadoso e atento neste campo de inúmeras possibilidades.


Gestalt therapy constitutes itself as a dialogical psychotherapy, a clinic of engagement.Empirical research on the dialogical existentialism-gestalt psychotherapy interface is scarce.Considering the dialogical relation as the axis of this approach, the objective was to understand the experiences of psychology professionals regarding the dialogical relation in gestalt therapy.As a methodological resource, we used the qualitative research of a phenomenological method, with a semiotic orientation.Open phenomenological interviews were carried out with five collaborators.The data were recorded and transcribed in full.The analysis was based on phenomenological reflexivity, through description, reduction, and interpretation.Results indicate the opening, presence, and use of epoché by the therapist as fundamental elements for the dialogical relation.Positive I-You attitude experiences have been found, suggesting culminating episodes of this process.There was confusion between the concept of I-You attitude and I-You moments, with an appreciation of this principle to the detriment of the I-It attitude, which evidences the need to rethink the formation of the gestalt therapist. Thus, the dialogical relation in gestalt therapy presents itself as a field of phenomenal pluralities.It is up to the professional to thread carefully and attentively in this field of innumerable possibilities.


La Gestalt-terapia se constituye como una psicoterapia dialógica, como una clínica del encuentro. Investigaciones empíricas sobre la interface existencialismo dialógico-psicoterapia gestáltica son raras. Considerando la relación dialógica como eje de este abordaje, se ha objetivado comprender las experiencias de profesionales de psicología con respecto a la relación dialógica en Gestalt-terapia. Como recurso metodológico se ha utilizado la investigación cualitativa de método fenomenológico, de orientación semiótica. Han sido realizadas entrevistas fenomenológicas abiertas con cinco colaboradoras. Los datos han sido grabados y transcritos en la íntegra. El análisis se ha pautado en la reflexividad fenomenológica, con un recorrido hacia la descripción, reducción e interpretación. Los resultados indican la apertura, presencia y el uso de la epojé por el terapeuta como elementos fundamentales para la relación dialógica. Experiencias positivas de actitud Yo-Tú han sido encontradas sugiriendo episodios culminantes de este proceso. Hubo confusión entre el concepto de actitud y momentos Yo-Tú, con la valorización de este principio en detrimento a la actitud Yo-Eso, lo que evidencia la necesidad de repensar la formación del Gestalt-terapeuta. Así, la relación dialógica en Gestalt-terapia se presenta como un campo de pluralidades fenoménicas. Cabe al profesional un andar cuidadoso y atento en este campo de innúmeras posibilidades.

The aim of this study was to qualitatively analyze and compare the written responses clients in treatment for posttraumatic stress disorder (PTSD) gave when asked about helpful and negative aspects of the previous session. Data were collected during a randomized quantitative study comparing dialogical exposure therapy (DET, a gestalt-based integrative therapy, n = 58) and cognitive processing therapy (CPT, a cognitive-behavioral therapy, n = 52). We developed a coding manual using Mayring's (2015) qualitative content analysis approach and were able to code responses reliably. Five main categories emerged, unspecified and specific therapy elements, therapeutic relationship, client process, and general positive experience, with the first three containing subcategories. Each therapy had its own codes for specific therapy elements, while the other codes were identical for both treatments. The results for specific therapy elements were in accordance with the theories behind the two treatments, with CPT clients naming cognitive factors most frequently, whereas in DET, emotions were the most frequently mentioned. Results for therapeutic relationship were surprisingly similar given that DET therapists consider the therapeutic relationship to be an active ingredient, while CPT therapists consider it as the basis for the intervention rather than a working mechanism in itself. RESUMO

This study examined the impact of gestalt intervention groups for anxious Chinese parents in Hong Kong. A non-randomized control group pre-test/post-test design was adopted. A total of 156 parents participated in the project. After 4 weeks of treatment, the intervention group participants had lower anxiety levels, less avoidance of inner experiences, and more kindness towards oneself and mindfulness when compared to control group participants. However, the dimension of self-judgment remained unchanged. The adaptation of gestalt intervention to suit the Chinese culture was discussed.

We analyzed the effectiveness of two psychological therapies to treat depression in the Culiacan population, Mexico. According to criteria of MINI (international Neuropsychiatric interview), 30 individuals from a total of 300 were selected and diagnosed with some kind of depression. Patients were divided in three groups: 1) treatment with hypnosis therapy, 2) treatment with Gestalt-hypnosis therapy, and 3) control group. Before and after the treatments the Beck Anxiety Inventory (BAI) was applied to know the depression level of the analyzed groups. The results show that the three groups were presenting a moderated level of depression. The groups under hypnosis therapy and Gestalt-hypnosis therapy show statistical differences between pre-test and post-test. The hypnosis therapy shows significant statistic differences to treat depression with respect to the other two groups. In conclusion, the therapeutic hypnosis is an effective treatment and has relevance to treat depression, while other therapeutic treatments tend to be slow and with minor result. This study is the first of this kind carried out in Culiacan in Sinaloa, Mexico


En este estudio se analiza la eficacia de dos terapias psicológicas para tratar la depresión en la población de Culiacán, México. Según los criterios de la MINI (entrevista neuropsiquiátrica internacional) se seleccionó a 30 sujetos de un total de 300 y se les diagnosticó de algún tipo de depresión. Se dividió a los pacientes en tres grupos: 1) tratamiento con terapia hipnosis, 2) tratamiento con terapia gestalt-hipnosis y 3) grupo control. Antes y después de los tratamientos se aplicó el Inventario de Ansiedad de Beck (BAI) para conocer el grado de depresión de los grupos analizados. Los resultados muestran que los tres grupos presentaban un grado moderado de depresión. Los grupos de hipnosis y gestalt-hipnosis muestran diferencias estadísticas entre el pretest y el postest. La terapia de hipnosis muestra diferencias estadísticamente significativas para tratar la depresión con respecto a los otros dos grupos. En conclusión, la hipnosis terapéutica es un tratamiento eficaz y es relevante para tratar la depresión, mientras que otros tratamientos terapéuticos tienden a ser más lentos y tener peores resultados. Se trata del primer estudio de este tipo llevado a cabo en Culiacán, Sinaloa, México

Cuando en 1934 el debate sobre la técnica terapéutica se convirtió en el eje de la investigación psicoanalítica, Hellmuth Kaiser publicó Problemas de técnica. Su propuesta técnica se basaba en el análisis sistemático de las resistencias, la técnica del «análisis del carácter», llevándolo mucho más lejos que su maestro Wilhelm Reich, hasta el extremo de prescindir de las interpretaciones de contenido y, a través de la recuperación del factor catártico, lograr que las interpretaciones fueran producidas por el analizando. El texto de Kaiser tuvo enorme impacto y recibió las críticas de algunos de los principales investigadores de la técnica psicoanalítica (Reich, Fenichel y Alexander). El rechazo a sus propuestas, unido a su interminable exilio y su prematura muerte, hicieron que su obra cayera en el olvido. Actualmente sus trabajos son reconocidos desde perspectivas psicoterapéuticas muy alejadas unas de otras: por un lado, se reivindica su papel de precursor de la técnica de la Psicología del Yo, y por otro algunos autores encuadrados en la Psicoterapia Gestalt le reconocen como uno de sus precursores. En este artículo presentamos por primera vez en español la traducción de Problemas de técnica (1934), contextualizado históricamente y acompañado de un breve esbozo biográfico del autor (AU)


Around 1934 the debate on therapeutic technique became the focus of psychoanalytic research. In this context Hellmuth Kaiser, a Wilhelm Reich's disciple, published Problems of technique. His technical proposal was based on the systematic analysis of resistance, the technique of "Character analysis", taking it much further than Reich, to the extent of ending up without content interpretation and, through the recovery of the cathartic factor, encouraged the analyzand to produce his own interpretation. The text of Kaiser had an enormous impact and received criticism from some of the leading researchers of psychoanalytic technique (Reich, Fenichel and Alexander). The rejection of his proposals, together with its interminable exile and his untimely death, made his work fell into almost total oblivion. Today his works are recognized from psychotherapeutic perspectives far removed from each other: on one hand, he claimed his role as precursor of the ego psychology's technique, and on the other hand, some framed authors in Gestalt Psychotherapy recognize him as one of their precursors. In this paper, we present for the first time in Spanish the translation of Problems of technique (1934 ), historically contextualized and accompanied by a brief biographical sketch of the autor (AU)

BACKGROUND: Although there are effective treatments for posttraumatic stress disorder (PTSD), there is little research on treatments with non-cognitive-behavioural backgrounds, such as gestalt therapy. We tested an integrative gestalt-derived intervention, dialogical exposure therapy (DET), against an established cognitive-behavioural treatment (cognitive processing therapy, CPT) for possible differential effects in terms of symptomatic outcome and drop-out rates. METHODS: We randomized 141 treatment-seeking individuals with a diagnosis of PTSD to receive either DET or CPT. Therapy length in both treatments was flexible with a maximum duration of 24 sessions. RESULTS: Dropout rates were 12.2% in DET and 14.9% in CPT. Patients in both conditions achieved significant and large reductions in PTSD symptoms (Impact of Event Scale - Revised; Hedges' g = 1.14 for DET and d = 1.57 for CPT) which were largely stable at the 6-month follow-up. At the posttreatment assessment, CPT performed statistically better than DET on symptom and cognition measures. For several outcome measures, younger patients profited better from CPT than older ones, while there was no age effect for DET. CONCLUSIONS: Our results indicate that DET merits further research and may be an alternative to established treatments for PTSD. It remains to be seen whether DET confers advantages in areas of functioning beyond PTSD symptoms.

As an older woman therapist, I find that my life experience grounds me in my work with people of all ages and backgrounds as they deal with life crises, aging issues, and loss. People with whom I work in therapy appreciate the fact that I am older and have had varied life experiences. Gender issues are still central to my work with clients whether I am working with a man or a woman. I am an integrative therapist, with a background in cognitive-behavioral therapy and gestalt therapy. Therapists need to help clients to identify less with their aging bodies and our culture's view of attractiveness, shifting instead to a paradigm that values life experience and the cultivation of wisdom. We need to find ways of embracing what we have learned about life instead of extolling youthful values. As I get older, I more fully appreciate a constructivism framework and life-cycle perspective, focusing on making sense of clients' life narratives. Storytelling and memoirs have both provided a framework for working with clients on coping with the many changes and challenges of life that bring them to therapy and added another layer to my integrative therapeutic work.

RESUMO

This is a report of a short-term, pastoral counselling group conducted with Jewish internees in a high security prison in Israel. It was held as an adjunct to daily secular individual and group counselling and rehabilitation run by the Department of Social Work. Pastoral counselling employed spiritual and psychosocial methodologies to reduce anger, improve prisoner frustration tolerance, and develop a sense of self-efficacy and communal identity. It combined semi-didactic scriptural input with Pierre Janet's personality model, Fritz Perls' gestalt therapy, and analysis of the group process.

Dynamics of experience as such and its corporeal, emotional and cognitive elements in the situation of psychological consulting provisioning is covered. The aim of research was to study psychological crisis experience dynamics in the situation when psychological consulting by gestalt therapy methods is provided. Theoretical analysis of the problem of crisis situations, phenomenon and structural, and dynamic organization of experience of the subject of consulting have been carried out. To fulfill research project test subjects experience crisis situation have been selected, studied in the situation when they provided psychological consulting by methods of gestalt therapy, and methodology of study of crisis situations experience has been prepared. Specifics of psychological crisis experience have been revealed and its elements in different stages of psychological consulting by gestalt therapy methods. Dynamics of experience of psychological crisis and its structural elements have been revealed and reliable changes in it have been revealed. Dynamics of psychological crisis experience and its structural elements have been revealed and reliable changes in it have been revealed. "Desiccation" of experience is being observed, releasing its substantiality of negative impression to the end of consulting and development of the new experience of control over crisis situation. Interrelations of structural elements of experience in the process of psychological consulting have been shown. Effecting one structure causes reliable changes in all others structural elements of experience. Giving actual psychological help to clients in crisis situation by methods of gestalt therapy is possible as it was shown in psychological consulting sessions. Structure of client's request has been revealed - problems of personal sense are fixed as the most frequent cause of clients' applications, as well as absence of choices, obtrusiveness of negative thoughts, tend to getting stuck on events took place in the past, drawing into oneself, etc.

Um dos expoentes da Psicologia da Gestalt que mais enfatizou a necessidade do método fenomenológico na psicologia foi Köhler. Para ele, o método fenomenológico permite abordar adequadamente aquilo que ele denomina a "experiência direta", sem cometer os erros oriundos do introspeccionismo clássico. O objetivo deste artigo é detalhar o método fenomenológico de Köhler no âmbito de sua teoria da percepção. Ao fim, pretende-se mostrar que ele é central para a fundamentação da Gestalt, por servir como ferramenta para distinguir o objeto da física do objeto da experiência direta.(AU)


Köhler was one of the authors of Gestalt psychology that most emphasized the need of the phenomenological method. For him, the phenomenological method allows properly approach the "direct experience" without committing the errors derived from classical introspection. The purpose of this article is to detail the phenomenological method of Köhler in his theory of perception. At the end, we intend to show that it is central to the fundaments of the Gestalt psychology. The phenomenological method serves as a tool to distinguish the direct object of external experience from the physical object.(AU)


Köhler fue uno de los autores de la psicología Gestalt que más insistieron en la necesidad del método fenomenológico. Para él, el método fenomenológico permite abordar adecuadamente la "experiencia directa" sin cometer los errores derivados de la introspección clásica. El propósito de este artículo es detallar el método fenomenológico de Köhler en su teoría de la percepción. Al final, tenemos la intención de mostrar que es fundamental para los fundamentos de la psicología de la Gestalt. El método fenomenológico sirve como una herramienta para distinguir el objeto directo de la experiencia externa del objeto físico.(AU)

RESUMO

OBJECTIVE: The research on psychotherapy for posttraumatic stress disorder (PTSD) stems predominantly from a cognitive-behavioral orientation while other approaches are underrepresented. We evaluated dialogical exposure in trauma therapy (DET), a treatment for PTSD combining cognitive-behavioral elements with an interpersonal, gestalt-based framework. METHODS: In this uncontrolled pilot trial, 25 PTSD patients were treated with DET in an outpatient setting and 21 completed therapy. RESULTS: There was a significant reduction in self-rated PTSD symptoms from pre- to posttreatment. Effect sizes were large in the completer sample and moderate to large in the intent-to-treat sample. General psychopathology also decreased significantly. The dropout rate was rather low at 16%. CONCLUSION: These results show that further research on DET as a treatment for PTSD is warranted.

Este estudo qualitativo descritivo em saúde articula a arte na modalidade de produção plástica e a psicoterapia gestáltica, no contexto da intervenção precoce com bebês pré-termo em hospitalização. Objetivou-se favorecer o processo de ressignificação do bebê, ao identificar as dificuldades subjetivas na relação família-bebê pré-termo; estimular os familiares à compreensão do bebê enquanto sujeito; e facilitar a emergência do potencial criativo familiar frente à prematuridade. Para tanto, utilizou-se o registro descritivo de sete sessões de grupoterapia, com 79 acompanhantes e 61 bebês, do banco de dados do serviço de psicologia da Unidade de Cuidados Intermediários Neonatais Canguru de um centro de referência nacional, de fevereiro a abril de 2012. A análise fenomenológica interpretativa dos dados revelou que a terapia pela arte favorece um espaço potencial para o bebê ser posicionado por sua família como sujeito, viabilizando a relação dialógica, com novas significações em torno do recém-nascido pré-termo. Portanto, essa psicoterapia de grupo na unidade neonatal surge como uma proposta de intervenção à psicologia no contexto de fortalecimento do Método Canguru no Brasil.(AU)

Aimed to facilitate the process of baby reframing, identifying the subjective difficulties in the family-preterm baby; to encourage families to anticipate the baby as a subject in training; and facilitate the emergence of creative family potential regarding prematurity. For this, was used the description of seven group therapy sessions, with 79 companions and 61 babies, the database of psychology service of Neonatal Intermediate Care Unit Kangaroo, a national referral center, from February to April 2012. The phenomenological interpretative analysis of the data revealed that the art therapy favors a potential space for the baby to be placed by his family as a subject in training, enabling a dialogical relationship with new significations around pre-term newborn. Therefore, this group psychotherapy in the neonatal unit appears as an intervention proposal to psychology in the context of strengthening of Kangaroo Care in Brazil.(AU)

RESUMO

Compared to other intervention modalities, there are relatively few outcome studies of gestalt therapy. This study aimed to address this gap by evaluating the therapeutic effects of four gestalt-oriented group workshops designed to enhance well-being in professionals trained as social workers, counselors, and psychologists. The results of this preliminary, uncontrolled study indicated that the 55 participants had better emotional well-being and experienced a heightened sense of hope following the workshops. The factors leading to these positive outcomes are discussed.

Psychiatric nurses interested in extending their interpersonal and psychotherapeutic skills sometimes undertake postgraduate training in gestalt therapy. Little is known about how this new knowledge and psychotherapeutic skill base informs their practice. This paper presents the findings of a qualitative study that aimed to explore the influence of gestalt therapy training on psychiatric nursing practice. Within a framework of narrative inquiry, four psychiatric nurses trained in gestalt therapy were invited to tell their stories of training in a gestalt approach to therapy, and recount their experiences of how it influenced their practice. In keeping with narrative analysis methods, the research findings were presented as a collection of four stories. Eight themes were derived from a thematic analysis conducted within and across the four stories. The discussion of the themes encapsulates the similarities and differences across the storied collection, providing a community and cultural context for understanding the individual stories.

RESUMO

The article investigates the relationship between crucial concepts and understandings in gestalt therapy and cognitive therapy aiming at discussing if and how they can be mutually enriching when considered as complementary parts in a more encompassing integrative therapeutic approach. It is argued that gestalt therapy, defined as a field-theoretical approach to the study of gestalt formation process, can complement the schema-based understanding and practice in cognitive therapy. The clinical benefits from a complementary view of the two approaches will be a wider scope of awareness toward individual and contextual aspects of therapeutic change processes, toward different levels of memory involved in these processes, and toward the relationship between basic needs, sensation and cognition in therapeutic work. Further, a dialogue between the two approaches will pave the way for addressing the connection between fundamental awareness work in gestalt therapy and the tendency within cognitive therapy toward incorporating mindfulness as a therapeutic tool. In the conclusion of the article, additional complementary points between the two approaches are outlined.
